Nobody compares the routes to an MSO and friendly-PC structure, so this page does: eight healthcare law firms with telehealth practices, five productized formation services with each one's not-a-law-firm posture stated (disclaimers quoted verbatim where they exist), the platform-included route where the structure comes with the software, and the DIY route with its risk stated plainly. Published pricing where it exists (three of the thirteen listed services post real figures), quote-only stated as such, and two firms from our candidate list that turned out to have closed, recorded so you don't call them. (This page is operator orientation as of September 2026, not legal advice; corporate-practice rules are state law in motion, and you'll want healthcare counsel for the real thing.)

Bespoke counsel: you own everything, the structure is built for your exact states and model, and almost nobody publishes fees (the attorney-advertising norm). Descriptions below are each firm's own; this page recommends none of them.

Profiles reflect each vendor's public positioning as verified on September 8, 2026. Two candidate firms were verified closed during this pass and are not rowed: Cohen Healthcare Law Group states on its own homepage that it stopped providing legal services as of July 17, 2026 (Michael H. Cohen joined Offit Kurman, per that firm's July 20, 2026 announcement), and Fenton Jurkowitz has closed with its healthcare attorneys continuing at Much. Checked but unreachable at review: Chapman Law Group, MedLink (both flagged for a second look). This page is re-verified quarterly and on any closure, merger, or rename touching a listed vendor.

Route 3: the structure that comes with the platform.

On an operated platform, the clinical side arrives with the software, and in some versions the medical-group arrangement itself does too, so the founder never forms a PC or hires a PC owner. The verified pattern from vendors' own public pages, September 8, 2026: platforms consistently advertise clinician provision, but almost none put entity provision in writing. Beluga describes the doctor network and clinical infrastructure that sits behind your brand; Karpa states outright that you do not need a medical license to run a white-label clinic on its platform, with partnership from-pricing on its offers page (both paid tiers display from $297 a month, while its main pricing page carries no figures). OpenLoop and Wheel state clinician provision without entity language, and Bask is the software-only counter-example: no clinical entity stated at all. The lesson is that white-label platform never automatically means structure included; ask in writing who owns the professional entity. And route 4, DIY with templates: our pass found no credible standalone MSO-template shop for telehealth; the real template route runs through a law firm's membership (Lengea's tiers bundle templates with prepaid hours). The management services agreement is the load-bearing document, and a template that misprices the management fee or leaves control with the non-physician entity recreates the corporate-practice violation the structure exists to avoid. How much does it cost to set up an MSO and friendly-PC structure?

The published anchors as of September 8, 2026: Foundry PC posts $15,000 one-time formation plus $3,000 per month for the friendly PC owner; TeleLaunch posts a $50,000 flat build that includes the structure inside a full company launch; and Lengea Law publishes membership tiers at $399, $499, and $999 per month (billed annually, plus a $499 initiation fee) that bundle formation work with prepaid hours. Everyone else quotes: Permit Health after you share target states, MedPath and Zivian unpublished, and traditional law firms per engagement. State count, CPOM strictness, and whether you need a PC owner supplied are what move the quote.

Do I need a law firm, or can a productized formation service do it?

Read each service's own disclaimer, because they answer this themselves: Permit Health states it 'is not a law firm and does not provide legal advice', Foundry PC and MedPath carry equivalent language, and all three route the actual legal drafting through attorney partners or outside CPOM counsel. The productized route buys speed and a packaged process; the law-firm route buys bespoke judgment on your exact states and model. Either way a lawyer touches the documents; the question is whether one is advising you or their process.

What does 'platform-included' mean, and do platforms replace the structure?

On an operated platform, the clinical side (and in some versions, the medical-group arrangement itself) comes with the platform, so you never form a PC or hire a PC owner. The verified pattern across vendors' own pages: platforms consistently advertise clinician provision, but almost none put entity provision in writing publicly; Beluga and Karpa both publish explicit language about what sits behind your brand, while a software-only platform like Bask states no clinical entity at all, which is why 'white-label platform' never automatically means 'structure included'. Ask any platform, in writing, who owns the professional entity your patients are seen under.

Can I DIY the MSO structure with templates?

No credible standalone MSO-template shop for telehealth verified in our September 2026 pass; the closest real template route runs through Lengea Law's memberships, meaning through a law firm, not around one. The risk lives in the management services agreement: fee structures must hold up as fair market value, control boundaries must respect corporate-practice-of-medicine rules that vary by state, and a template that misprices the management fee or leaves control with the non-physician entity recreates exactly the violation the structure exists to avoid. DIY is the cheapest route until it isn't.
